When to change what you are doing for knee soreness

What to check before changing care
Check whether the current care was used often enough to judge. Regular gentle work tells more than scattered hard sessions. A brace left in the car can't help during a walk. Medicine stopped because of side effects needs review before another dose.
Use an ordinary week to judge the knee, not one bad afternoon.
Daily tasks give the clearest report. Note trouble reaching the mailbox, rising from a chair, or sleeping. An X-ray can add useful facts, but it doesn't choose the treatment. Tell the clinician about both the daily limits and any change in soreness.
When to ask about care before knee replacement
Joint preservation is QC Kinetix's term for considering non-surgical care before knee replacement. Its Chandler clinic offers PRP and concentrated PRP, both made from your blood. Staff take a small sample and spin it to collect more platelets. Platelets are blood cells that aid clotting. A provider then puts the prepared liquid into the knee. Decide which amount of relief would matter in your daily life.
Medical providers at the clinic check the knee before recommending care. They will ask about swelling, earlier procedures, medicines, and other health concerns. Take old reports when available. Ask how soon they will judge the first PRP treatment. The answer should name daily tasks such as walking or sleeping.
When to seek help without waiting
Certain knee changes require care before the next office opening. Fever beside a hot, red knee calls for urgent help. Go after an injury if the leg can't hold weight. Get prompt care if the knee sticks and cannot straighten.
Call quickly if the calf becomes warm or swollen.
For less urgent trouble, arrange an exam when the knee keeps giving way. Call if soreness often interrupts sleep or movement drops quickly. Don't add more home remedies while the knee worsens. A full exam is the safer next step.

At 5 years, the advantage of a supervised 12-week non-surgical programme over written advice in patients not eligible for knee replacement had disappeared (KOOS4 difference 5.3, 95% CI -1.5 to 12.1). Most patients in both arms improved in a clinically important way (76% of the supervised group and 66% of the written-advice group), and 15 of 50 (30%) versus 17 of 50 (34%) had received a knee replacement in the index knee by then.

A network meta-analysis of 149 randomised knee-osteoarthritis trials evaluating different placebo types found that intra-articular placebo produced a significant pain effect of its own (effect size 0.29, 95% credible interval 0.09 to 0.49) relative to oral placebo - meaning any uncontrolled injection appears to work partly because it is an injection, and open-label injection results systematically overstate benefit.

In a 2-year double-blind randomised trial in 140 patients with symptomatic knee OA (Kellgren-Lawrence grade 2 or 3) and ultrasonic synovitis, intra-articular triamcinolone 40mg every 12 weeks produced significantly greater cartilage volume loss than saline (index-compartment cartilage thickness change -0.21mm vs -0.10mm; between-group difference -0.11mm, 95% CI -0.20 to -0.03) with no significant difference in pain.

When an exam would help
An exam can help when the knee stays sore or limits normal errands. Take any X-rays and a current medicine list. Note the soreness start date and which activities ease or worsen it.
